How I Approach Lab Testing
I don't hammer you with $1,000 in labs on day one. We're systematic. We start with what's most common and most foundational, then escalate if needed.
Baseline for almost everyone:
Metabolic markers: fasting insulin, fasting glucose, HbA1c, lipid panel
Expanded thyroid panel: free T3, free T4, total T3, total T4, TSH
Basic inflammatory markers: high-sensitivity CRP
Complete blood count (CBC), comprehensive metabolic panel (CMP)
This gives me a clear picture of metabolic health, thyroid function, inflammation, and basic organ function. For a lot of patients, this is where the answer lives.
If something doesn't add up, we go deeper:
Reverse T3, thyroid antibodies (TPO, TG)
Hormone panels (testosterone, estradiol, progesterone, DHEA-S, cortisol)
Advanced inflammatory and autoimmune markers
Nutrient deficiencies (vitamin D, B12, folate, iron panel, magnesium)
Viral reactivation markers (EBV, HHV-6, CMV)
Specialized functional medicine testing (gut, food sensitivities, toxins)
The principle is simple: start with what's most likely, then expand based on clinical suspicion and your symptoms. We're cost-conscious. We're thorough. But we're not wasteful.

Why I Order Labs Differently Than Most Doctors
I order what you need, not what insurance approves. Insurance-based medicine limits testing to what's "medically necessary" by their definition. That usually means waiting until you're sick enough to diagnose a disease. I'm trying to catch and fix dysfunction before it becomes disease.
I interpret labs in context, not in isolation. A TSH of 2.8 might be "normal," but if your free T3 is low and you're exhausted, that's not optimal. A fasting glucose of 95 is "normal," but if your fasting insulin is 18, you're insulin resistant. Context matters.
I look for patterns, not just individual numbers. Your thyroid antibodies are positive, your CRP is elevated, your vitamin D is low, and you have gut symptoms — that's a pattern. That tells me something about immune dysregulation and inflammation. One abnormal marker is a data point. Multiple abnormal markers tell a story.
I retest to track progress. Labs aren't a one-time snapshot. We retest every 3-6 months (depending on the issue) to see if treatment is working, if values are trending in the right direction, if we need to adjust the plan.
